Assertive Community Treatment (ACT)

What is ACT?
ACT is a program that provides intensive case management to 50 individuals utilizing a multi-disciplinary team approach to provide direct services to a small well-defined caseload. The program focuses on reducing hospitalizations, ensuring basic needs are met, and maintaining engagement in treatment while promoting harm reduction. Treatment duration is not time-limited.
