How do I join?

Setting up your account is fast and easy. It takes just three steps:

  1. Make sure we have your email address. We should have gotten it from you at your first appointment. If you’re unsure if the email we have on file is up to date, call us at (847) 377-8592. 
  2. Check for an email from [email protected]. Follow the steps to sign up.
  3. Log in with the username and password you created. You’ll now have access to your health information anytime, anywhere.

Is the Patient Portal secure?

Yes. The Patient Portal is HIPAA-compliant. This means your information is securely stored and encrypted.

How do I contact my care team?

Click the “Messages” tab. Here, you can send a new message, attach images, and view your care team’s replies.

What should I do if I can't find the health records I'm looking for?

What do I do if I forgot my username and/or password?

Click “I forgot my username and/or password” below the sign-in button. Answer your security question and reset your password.
