The Community Health Survey helps the Health Department gain insight into factors that may impact the community's current health and how they may be improved. The data collected will help us set goals and assist in implementing the Community Health Improvement Plan (CHIP).

The data not only helps us know how we can improve Lake County, but it gives you a chance to express what matters to you. Do I need to provide identifying information?
No. The survey is completely anonymous. It will only ask basic demographical questions (race/ethnicity, age, zip code). You will not be asked to provide your name, phone number, email, or social security information.
Has this survey been provided in the past?
Yes. Versions of this survey have been provided in the past. However, as the problems and concerns that are facing Lake County are always evolving the survey, also changes to make sure we capture the most important information.
How long is the survey?
The survey consists of 40-50 questions. The survey will take about 15-20 minutes to complete.
