I understand by using the Illinois Mail-in Voter Registration form without providing sufficient proof of identity that the first time I vote must be in-person at either my Election Day or any early voting site.

I further understand that if I supply my email address on the application, Illinois election code requires election officials to provide my email information to campaign committees and political organizations.
