Community Health Improvement Plan (CHIP)

The Community Health Improvement Plan (CHIP) is a long-term, systematic effort to address public health problems in a community. The plan is based on the results of Community Health Assessment (CHA) activities and is part of a community health improvement process.

Community Health Assessment (CHA)

The Community Health Assessment (CHA) uses quantitative and qualitative methods to collect and examine health status indicators and provide an understanding of health in a community. The CHA guides the development and implementation of a Community Health Improvement Plan (CHIP) by justifying how and where resources should be allocated to best meet community needs.

The Mobilizing for Action through Planning and Partnerships (MAPP) framework was utilized for the assessments.
