- Community Health Improvement Plan Community Health Improvement Plan (CHIP) is a long-term, systematic effort to address public health problems in a community. The plan is based on the results of CHA (Community Health Assessment) activities, and is part of a community health improvement process. MAPP (Mobilizing for Action through Planning and Partnerships) is a community-driven strategic tool utilized for conducting the community health improvement process.
- Health Data Profiles A health data profile can be an overview, summary, or report of a particular health issue, such as cancer. Or it can address a set of issues, such as the health issues of a community’s adolescents. It may also focus on select key indicators of a community’s health, such as health behaviors like tobacco use or healthful eating.
- Strategic Plan and Annual Reports Strategic planning is a process for defining and determining an organization’s roles, priorities, and direction over three to five years. A strategic plan sets forth what an organization plans to achieve, how it will achieve it, and how it will know if it has achieved it. The strategic plan provides a guide for making decisions on allocating resources and on taking action to pursue strategies and priorities.
