About Us
The Public Health Nursing/Family Case Management Program offers home visits by a registered nurse or case manager to low income families with pregnant women, infants and high risk children up to age two. The purpose of the visits is to provide advocacy, coordination of medical, dental and social services. Health assessments, counseling, education and screenings are provided as well as referrals to area resources such as health care providers, transportation, nutrition services, and mental health services.


Who Can Receive Services
Pregnant mothers with preschoolers age 3 to 5 or a mother of an infant who also has a preschooler age 3 to 5. The family must currently be receiving Family Case Management services through the Health Department and  live in the Waukegan, North Chicago or Zion Elementary Public School Districts.
