About Us
The Public Health Nursing/Family Case Management Program offers home visits by a registered nurse or case manager to low income families with pregnant women, infants and high risk children up to age two. The purpose of the visits is to provide advocacy, coordination of medical, dental and social services. Health assessments, counseling, education and screenings are provided as well as referrals to area resources such as health care providers, transportation, nutrition services, mental health services, substance abuse treatment services.
Who Can Receive Services
Clients must be residents of Lake County and generally need to be WIC active. They must have an income less than 200 percent of the federal poverty level.

Public Health Nurses also provide information to high-risk Lake County residents on how to access many programs available through the Health Department and Community Health Center, including well-baby clinics, immunizations, prenatal services, family planning, WIC and job training.
