About Us
This program makes primary and specialty medical care available and accessible to children in foster care. Medical records are gathered and maintained for all wards of the state. Services provided include helping foster parents find medical care providers, evaluating the medical status and needs of the foster child and giving the parents a Health Works passport, which contains a summary of the child's medical history.
Who Can Receive Services
Clients served must be Department of Children and Family Services wards or adolescent wards who are pregnant or parenting.
