HIV/AIDS Services

We provide individuals diagnosed with HIV/AIDS with a 360-degree visit so that his or her physical, emotional and lifestyle needs are addressed at each visit.

Services Available

  • Adherence support
  • Group support in English and Spanish
  • Health education
  • Linkage to community support and social services, such as legal services, food, housing, and substance use treatment
  • Medical care
  • Nutritional assessment and counseling
  • Dental care, including routine preventative, emergency, and restorative
  • Pharmaceutical assistance
  • Insurance assistance
  • Case management
  • Transportation assistance
  • Referrals for specialty care and diagnostic testing

Eligibility

  • Any person diagnosed with HIV/AIDS
  • Financial support for medical services is available through a grant for individuals:
    • With or without insurance 
    • Living below 500% of the Federal Poverty Level
