If, after contacting us, you do not feel that your concerns regarding safety or quality of care have been sufficiently addressed, you may contact the Joint Commission on Accreditation of Healthcare Organizations at 800-994-6610 or by email.

Federal Tort Claims Act (FTCA) Protection

The Lake County Health Department and Community Health Center receives funding from the U.S. Department of Health and Human Services. As such, the Health Department has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims. This includes medical malpractice claims for itself and its covered individuals.
