Homeless Assistance (Emergency Solutions Grant & Continuum of Care)

Point-in-Time Count Summary Report

The Lake County Community Development Division, in cooperation with the Lake County Coalition for the Homeless, conducts an annual Point-in-Time Count. The count, which is usually conducted in the last week of January, produces a snapshot census of the homeless population. Data from the count is available in summary reports.

Lake County has grant funds available annually for non-profit organizations and governmental entities to assist homeless persons. These funds provide for emergency shelters, transitional housing, permanent housing, and supportive services.

The Lake County Coalition for the Homeless works to meet the requirements of a Continuum of Care (CoC) Program including coordination of homeless services, promotion of efforts to end homelessness and the submission of an annual funding application. The Homeless Coalition meets from 12:30-2:30 on the first Tuesday of every month. Both groups meet at the United Way.

One of the key accomplishments of these groups is the development of a 10 Year Plan To End Homelessness in order to mobilize, advocate and empower community collaboration towards the elimination of homelessness in Lake County.

Recipients of funding for homeless assistance must participate in a Homeless Management Information System (HMIS). Please visit the HMIS page for more information.

The County is required by the HEARTH Act to establish written standards 
for the provision of Emergency Solutions Grants (ESG). The following policies establish these standards - Policy Standards and Procedures

The application for CoC funds is available from the Lake County Coalition for the Homeless. This is a nationwide competitive grant process. Please contact Community Development staff or the Lake County Coalition for the Homeless for details.
