The Circuit Clerk’s Office accepts applications for passports and processes
them through a Chicago passport agency. Forms to apply for a passport
are available at the Circuit Clerk’s Office.
PLEASE NOTE: All persons must appear in person to apply for a passport.

Adult Passport Requirements
1. A certified copy of your birth certificate, from the county you were born. Hospital certificates are not accepted.
If you are a naturalized citizen, your original Naturalization Certificate will be needed. This document will be returned.
2. Two 2X2 passport photos. Photo booth photos are not accepted.
3. A valid Driver’s License or State ID card. If your Driver’s License is being held due to a traffic citation, you must obtain a State ID card before your application can be processed.
4. A completed passport application. Please DO NOT sign the application form until you are in front of the accepting agent.
Adult Passport Renewals
If your passport has been expired 5 years or less, you can renew it by mail. The Circuit Clerk’s Office carries the form, but you must send it in the mail.
If your passport has been expired more than 5 years, you must fill out a new passport application. Instead of your certified birth certificate, your old passport (which will be returned) is needed, as well as new passport photos, & a valid I.D. The same fees apply as a first time passport.
Minor Passport Requirements
All minors must be present for the passport application to be processed.
For minor children 15 years of age & under, both parents must be present. If one parent is not available, a Statement of Consent, along with a copy of the non-applying parents identification will be accepted. For minor children 16 years of age & over, only one parent need be present.
1. A certified copy of the minor’s birth certificate from the county the minor was born. Hospital Certificates are not accepted.
If the minor’s parent(s) are naturalized citizen(s), the original Naturalization Certificate(s) will be needed.
2. Two 2X2 passport photos. Photo booth & school photos are not accepted.
3. Both parents valid Drivers License or State ID card. If the Drivers License is being held due to a traffic citation, a State ID card must be obtained.
Minors over 16 years of age with an ID must present it to the accepting agent.
4. A completed passport application. Please DO NOT sign the application form until you are in front of the accepting agent.
