  • Demonstration Home Assessment Application  
    Builders can apply for preferential assessments for demonstration model homes.

  • Disabled Persons' Homestead Exemption Application 
    This exemption lowers the equalized assessed value of your property by $2,000, and may be claimed in addition to the Homestead Limited Exemption and the Senior Citizen's Homestead Exemptions, if applicable. (This is for first time applicants ONLY)

  • Disabled Persons' Homestead Exemption Annual Verification Application 
    After initial application and approval in the first year you qualify for this exemption, it is required that each year thereafter you renew the exemption using this verification form. This form is mailed each year to all persons who received this exemption in the previous year. (This should not be used by first time applicants)

  • Disabled Persons' Homestead Exemption - Physician's Statement  
    If you are unable to provide proof of your disability as shown on the Disabled Persons' Homestead Exemption application, you must submit this form. It must be completed by a licensed physician in the State of Illinois - you will be responsible for any costs incurred for your examination. This form also must be accompanied by either the Disabled Persons Homestead Exemption Application or Annual Renewal Application.

  • Disabled Veterans' Standard Homestead Exemption Application 
    This lowers the equalized assessed value of your property by $2,500 if your disability is service-connected and you are at least 50% disabled, but less than 70% disabled.  This exemption lowers the equalized assessed value of your property by $5,000 if your disability is service-connected and you are at least 70% disabled.  Note that this exemption is limited to residential property with an equalized assessed value of less than $250,000. (This should be used by first time applicants ONLY)

     
  • Disabled Veterans' Standard Homestead Exemption Annual Verification Application 
    After initial application and approval in the first year you qualify for this exemption, it is required that each year thereafter you renew the exemption using this form. The Chief County Assessment Office will automatically mail these forms to all persons who received this exemption in the previous year. (This should not be used by first time applicants)

  • Disaster Area Application For Reassessment  
    Because Lake County, IL was declared a disaster area by Governor Blagojevich on August 24, 2007, we have made this form available for those who may have sustained structural damage to their properties due to the recent storms and flooding. Personal property (washers, dryers, keepsakes, etc…) losses cannot be included, as this is intended for damage to actual real estate that is assessable for real estate tax purposes. This form can be submitted to either your local township assessor or the Chief County Assessment Office, and must include documentation to support the application such as appraisals, contractor's estimates, or estimates by a governmental body (i.e., FEMA). If there should be actual structural damage and a reduction in assessment is warranted, the reduction will be pro-rated to the date of the Governor's proclamation.

  • Fraternal Organization Assessment Freeze Application 
    Application and Affidavit for IRC 501 (c)(2), (c)(8) or (c)(10) Fraternal Organization Assessment Freeze

  • Returning Veterans' Homestead Exemption Application 
    This exemption lowers the equalized assessed value of your property by $5,000 in the year you return from active duty in an armed conflict and the following year after.  This can be claimed in addition to the General Homestead Limited Exemption and any applicable Senior Citizen's Exemptions.

  • Senior Citizen Assessment Freeze 2012 Application  
    Taxpayers who are 65 or older during 2012, own property as your principal residence for the last two Jan. 1st, and have a 2011 total household income of $55,000 or less will be eligible for the assessment freeze for tax year 2010.

  • Senior Citizen Assessment Freeze 2011 Application
    Taxpayers who are 65 or older during 2011, own property as your principal residence for the last two Jan. 1st, and have a 2010 total household income of $55,000 or less will be eligible for the assessment freeze for tax year 2011.

  • Senior Homestead Exemption Application
    Seniors who are at least 65 years of age are eligible for a homestead exemption.
    This is filed with your local Township Assessor's office, and requires proof of age and ownership. Please contact your township office prior to filing to find out exactly what requirements they have. If applying by mail, proof of age and ownership must be included with the form.

  • Valuation of Property Used for Open Space 
    This form can be used to apply for preferential open space valuation on parcels of ten acres or more. This form needs to be filed by June 30th each tax year.

  • Veteran's Organization Assessment Freeze Application
    This application form can be used by veteran's organizations that meet specific criteria and would like their tax assessment frozen.
